We Offer the Following Services:

Our staff provide a variety of services to care for our patient’s acute and chronic needs from children to the elderly. 

Behavioral Health Counseling

Walk in/Emergent care. 

Preventive Care:

Children: Should have regularly scheduled well child checks starting at 1-2 weeks of age/newborn, 2 mo., 4 mo., 6 mo., 9 mo., 12 mo., 15 mo., 18mo., 24 mo., then yearly.

Children & Teens should continue yearly well child checks in addition to sports physicals.

Adults: Should have a yearly physical.

Gynecological and Prenatal care are provided.

Medicare Wellness Visits are provided for patients with Medicare.


Chronic Care:

If you have a chronic condition such as diabetes, high blood pressure, COPD, asthma, or high cholesterol you should visit your provider every 3-6 months (depending on your health status).  Your provider will advise you when to return.  If you are having complications, changing a medication, or not feeling well, you may be advised to return sooner.

Minor procedures such as biopsies and wart removals
Hearing and Vision screening
Spirometry Testing
Tetanus and influenza vaccinations
Lab testing & X-Rays
Allergy injections


We provide future and same-day appointments and ask that you contact us to schedule these.  If you are unable to keep your appointment please contact our office to cancel or reschedule.

We do our best to schedule you with your chosen regular provider, but due to the rotating schedule that may not always be possible if you need to be seen for a condition that cannot wait.

Medical insurance cards
Photo ID
Emergency contact information
All prescriptions, non-prescriptions, and herbal medications you are taking and/or a current list including dose and frequency for all your medications
A list of any allergies & reactions



During your visit

To best provide you with the quality healthcare we will review and update your medications and allergies at every visit. 

Your health history will be updated as needed with any changes to surgical, family, or social history. 

Education and self-management resources are available depending on your condition and needs.

To effectively care for your overall health we also coordinate your care with other medical facilities that may be treating you (hospitals, specialists, testing facilities, behavior health, dentistry).  We ask that you provide any names and contact information you may have from any other medical providers you are currently seeing.

Prescriptions & Refills

Please contact your pharmacy for a renewal and they will send the request to our office if needed.  Please call our office to schedule your next appointment before you run out of medication.  Medication refills are renewed during regular office hours. 

Sliding Discount

It is the policy of Challis Area Health Center, Inc., to provide essential services regardless a patient’s inability to pay. Discounts are offered on a sliding discount that is based off of the Poverty Level that is published by Health and Human Services each year for patients who meet the income requirements.
